Please complete the following steps (appropriate consent form and schedule appointment) below. If you have previously completed a consent form for HERO school project testing, you do NOT need to complete the consent form again.

Step One: Consent Forms:

• STUDENT: This form must be completed by a parent or guardian over the age of 18.
• FACULTY OR SCHOOL STAFF MEMBER: This form is to be completed by school staff members prior to testing.
